What data do we collect and why?

Data is obtained directly from registering with Forest Veterinary Clinic – this may be face to face, on the telephone, via email or using online registration.

When you register your pet with Forest Veterinary Clinic we will ask for personal data to allow us to provide you with veterinary services.  These services include, but are not limited to, professional advice, diagnosis, treatment and preventative care for your animals.  A minimal amount of personal data is also required to allow purchase of prescription medications.

The registered owner will be asked to provide a name, address and contact details.  With permission from the owner, the practice may contact any previous veterinary surgery in order to obtain clinical history for registered animals. This is necessary to ensure continuity of care. Likewise, if a client no longer wishes to use our services for whatever reason, with their consent, we will send clinical history to their chosen veterinary practice. The client may request that all personal data is deleted.

Use of Cookies

Tracking software is used to monitor website visitors to improve our understanding of how they make use of the website. This software is Google Analytics, which makes use of cookies to track visitor actions. Google Analytics will save a cookie to your device hard drive in order to track and monitor your engagement and usage of the website. It will not gather or store personal information. How is your data used? – Legitimate interests

Reminders for vaccinations or medications

·        External laboratories – for the purpose of laboratory tests

The name of the client is supplied in order to identify the sample being analysed. Contractual arrangement.

·        Veterinary referrals

With consent of the client, the referral hospital is provided with a clinical history, this will include name, address and contact information of the client.

·        Second opinions / Change of practice

With consent of the client, another practice may be provided with a clinical history, this will include name, address and contact information of the client.

·        Insurance companies

Under direction and consent of the client, clinical history including name, address and contact details of the client may be shared with an insurance company for the purpose of claiming veterinary fees.

·        Microchip companies

Client name, address and contact numbers are shared for identification purposes in order to find the owner of lost / unidentified animals.

What are your data protection rights?

Forest Veterinary Clinic would like to make sure you are fully aware of all of your data protection rights. Every client is entitled to the following:

The right to access – You have the right to request copies of your personal data. We may charge you a small fee for this service.

The right to rectification – You have the right to request that Forest Veterinary Clinic correct any information you believe is inaccurate.

The right to erasure – You have the right to request that Forest Veterinary Clinic erase your personal data, under certain conditions.

The right to restrict / object to processing – You have the right to request that Forest Veterinary Clinic restrict the processing of your personal data, under certain conditions.

The right to data portability – You have the right to request that Forest Veterinary Clinic transfer the data that we have collected to another veterinary practice, or directly to you, under certain conditions.

Right to judicial review – in the event that Forest Veterinary Clinic refuses your request for access, we will provide you with a reason.
